PARENT QUESTIONNAIRE

As part of whole school evaluation, it would be appreciated if you could complete the attached reply-slip.  The school serves the community and your opinion and feeling of “Our Own” is important to us.  Your feedback is very important for strategic planning and placing our school on the fore front of development.  This will benefit your child and our community.

Every question is answered by means of a tick in the columns marked 1-5, where 5 indicates that you are very satisfied and 1 that you are dissatisfied.

Thank you for your time and we trust that the feedback which we will receive, will be representative of our community.  We depend on your objective feedback, as we believe in an educational association with you.
