Dear Parent

CAMPAIGN : HELP ERADICATE POLIO AND ELIMINATE MEASLES

Officials of the National Department of Health will visit the school on Wednesday 14 April 2010 to immunise learners against measles and polio.  Please refer to the attached information letter in this regard.

However, it remains the parent’s choice whether or not a child will be immunised.  We therefore, kindly, request you to read the attached information and to return the reply-slip to school on or before 19 March 2010, so that the necessary arrangements can be made.

Should you not wish to have your child immunised, you may then only fill in your child’s name and cross out the rest.  We will then be able to make certain that all parents have read the information.
